2025-2026 Recreation Family Record
Participant Intake Form - This form is optional, but is very helpful in getting to know your child before they attend our programs.
Current Immunizations: If your child is not fully immunized, you must submit the following documentation annually through the Colorado Department of Public Health Website.
You will have the option to upload a copy of current immunization or exemption forms with your Recreation Family Record via DocuSign. Please have these ready and available when filling out your Recreation Family Record.
Medication Administration Form - This form is required if your child requires any medication to be given or carried, either by staff or the participant, during programming. This includes items such as EpiPens, inhalers, prescription medications, and all over the counter medication.

- 2025-26 Scholarship Online Application: Complete and submit this online form with the required documentation prior to registration for the scholarship to apply.
- Or use the 2025-26 Printable Scholarship Application: Print and fill out the application by hand and return with the required information to the Parks and Recreation office at 245 Howelsen Parkway prior to registration for the scholarship to apply. Please let staff know if you are also receiving funds through any other sources.
